There are yet more interesting forms concerning the tour to Spain to download on this page. The "Terms and Conditions" document was published previously. Also, a new "Intent to Travel"document has been published. This is a NHS Chorus document that needs to be turned in immediately if it is not now in the Spain folder in the chorus room. Sign the lastpage and returned it ASAP. Also new to the site is the latest, updated, "Itinerary". It too can be downloaded and printed. Another download includes the names of those traveling to Spain ("Travel List 2011"). A very important form is the "Registration" form. It asks for a copy of your passport. This will be the first collected by the NHS Chorus and then forwarded to Intropa. With this particular form is an "Insurance Application/Disclaimer" which must be completed in full to be accepted. Two additional forms that need your attention is the "Deviation" form that tells Intropa that you are not going to remain with the tour or that you are staying in Spain for an extended period and the "Land Only" form that allows you to buy your own airline ticket (you should do this quickly so you will have seats on the same flight as the rest of the group) and purchase the land portion of the trip separately from Intropa. Please take time to read each form and to complete and send the "Registration" (with a copy of your passport) and the "Intent to Travel" forms to the school as soon as you can.
